A poison shroom is an item that appears in Paper Mario: The Thousand-Year Door and Super Paper Mario.

The item could be made by Zess T. by mixing a Slow Shroom and Inky Sauce. When Mario or his partners use the item, they will lose half of their HP and will be affected with the Poison status. Strangly, in battle, there is a small chance that the item won't poison it's user, but heal all HP instead. The poison also appears as a slot picture. If Mario matches all three pictures, his HP, FP, and SP will be cut in half, and the audience members will leave.
Ghost T. died on the Excess Express after eating a Poison Shroom.
